I wish to enter the above item and agree to abide by the contest rules & divisions of the jury and judges. I send my quilt at my expense and pay \4,000, per entry, for the contest fee, return shipping & insurance. I understand that I need to value my work for customs declaration purposes only at no more than $50.00 (US) in order to avoid an expensive bond. Any additional duties and taxes are my responsibility to pay and I agree the Quilt Week Committee to charge my credit card if that happens. I understand that the Quilt Week Committee will take every precaution to protect my work exhibited in this show. I realize they cannot be responsible for the acts of nature beyond their control. You may have my permission to photograph this work. I understand that my signature gives the Quilt Week Committee the right to use a photo of my work in any publications, advertisements, or promotional materials.
*We encourage you to insure your work for its full value for insurance purposes, however, we must ask you to value it for customs declaration purposes only at no more than $50.00 (US). A higher custom value may require an expensive bond. If it happens, we must ask you to reimburse these expenses to us by your credit card or international money order issued by post office.
